综合医院精神科联络会诊6年的变化

【摘 要】目的:比较2008年与2014年综合医院精神科联络会诊的变化. 方法:回顾性分析、比较2008年及2014年武汉大学人民医院各科室邀请精神科会诊情况. 结果:精神科会诊患者2008年257例,2014年为688例;两年的会诊率、会诊患者的年龄、性别分布差异无统计学意义.与2008年相比,2014年患者单次会诊比例显著增加(Z =4.20,P＜0.05);内科、妇产科、儿科、五官科、肿瘤科会诊比例显著增高,外科、感染科和老年科会诊比例显著降低(P均＜0.05);患者因不能解释的躯体症状和既往精神障碍史但目前无症状的会诊比例明显增高,而急性脑综合征和精神病性症状的比例明显降低(P均＜0.05). 结论:6年间综合医院住院患者对精神科联络会诊需求无明显变化;非精神科医生对患者精神问题的关注增高,但识别能力不足.%Objective:To explore the variation of psychiatric consultation in general hospital in 2008 and 2014.Method:Retrospective analysis and comparison were conducted of the data of psychiatric consultation in all departments in Wuhan University People's Hospital in 2008 and 2014.Results:Respectively 257 cases and 688 cases in 2008 and 2014 invited the psychiatric consultation.The consultation rate,the distribution of age,gender of the psychiatric consultation'patients between this two years showed no statistical significance difference.Compared with 2008,the single consultation ratio of patients in 2014 increased significantly (Z


=4.20,P ＜ 0.05);the consultation ratios of internal medicine,gynecology and obstetrics,pediatrics,ophthalmology and

otorhinolaryngology,oncology departments increased significantly,but chirurgery,infectious and geriatrics departments were decreased significantly (all P ＜ 0.05);the ratios of consultation resons for patients with medically unexplained symptoms or a history of mental disorders increased significantly,but the reasons for acute brain syndrome and psychotic symptoms were decreased significantly (all P ＜

0.05).Conclusion:The demand of psychiatric contact consultation among the 6 years is no obvious change in hospitalized patients of general hospital.The doctors except for psychiatrist pay more attention to the mental problems of inpatients,but the ability to recognize is not enough. 【总页数】3页(P269-271)
